Before your first session, two forms are important from the list in the column to your left:

1. An Intake form.  You are invited to print this form and have it completed when you come for your first session. Instructions for printing are included with the form.

2. Please read the rules protecting privacy as you seek counseling.  Please bring the completed form printed at the end of the rules to your first session. This assures my records that you have read the rules. Instructions for printing the form are included with the form.

3. A Biographical Data Sheet form is also included.  Your counselor may as you to complete this form.  Its purpose is to ask that you write down a structured biography of your life to help understand and clarify events and perceptions in your history that may help your counseling.
